1. Mengisi Formulir Pendaftaran

2. Melunasi Administrasi Keuangan

3. Menyerahkan :

a. Surat keterangan pindah dari sekolah asal yang telah ditandatangani oleh Kepala Sekolah (asli)

b. Raport asli dari Sekolah asal

c. Nomor NISN siswa

       d. Menyerahkan photocopy dan softcopy Akte Lahir Asli

e. Menyerahkan photocopy dan softcopy Ijasah Asli SMP

f. photocopy dan softcopy STTB SMP (Sekolah Menengah Pertama) yang telah dilegalisir sebanyak 2 lembar

g. Surat Keterangan pindah dari Dapodik sekolah asal  / kemendibud (asli)

h. Photocopy NPSN Sekolah Asal  yang telah dilegalisir sebanyak 2 lembar

i. Photocopy Sertifikat Akreditasi Sekolah Asal  yang telah dilegalisir sebanyak 2 lembar

j. Surat Keterangan pindah dari dinas pendidikan kota asal (asli)

k. Photocopy dan softcopy Kartu Keluarga Asli sebanyak 2 lembar

l. Photocopy KTP Orang tua sebanyak 2 lembar
